Dawn L. Creech, 64, of Churubusco, passed away Saturday, July 18, 2026 at the Waters of Columbia City.

She was born in Wauseon, Ohio to the late Merle J. and Sandra Lee (Foote) Short on December 20, 1961.

Dawn worked as a team assistant for Transitions Hospice.

In her free time, Dawn enjoyed camping, being on the lake, and vacationing. Most of all, Dawn will be remembered for her profound love for her family, especially her children and grandchildren.

She is survived by her children, Toni (Errik) Polakovic and Tristan (Rebekah) Creech; and grandchildren, Skylee, Payslee, and Baylor.

Per Dawn’s wishes there will be nor formal services.
